CYC ID CARDS ARE REQUIRED FOR ALL KIDS:
SOCCER/SOFTBALL/BASEBALL-MUST HAVE PRIOR TO SOCCER IN 3RD GRADE
BASKETBALL/VOLLEYBALL-MUST HAVE PRIOR TO VOLLEYBALL IN 5TH GRADE

If your child plays both soccer and volleyball
you will need two cards in order to leave one with each coach.

PLEASE BRING ONE OF THE FOLLOWING FOR VERIFICATION OF BIRTH DATE:

BIRTH CERTIFICATE BAPTISMAL CERTIFICATE (Must list birth date on it)
CURRENT OR EXPIRED CYC ID CARD
PASSPORT
VALID DRIVERS LICENSE

CARD FEES:

YOUTH-UNDER 18 YEARS OLD-$3.00
ADULT-OVER 18 YEARS OLD-$3.00-Never expires

SOME RULES:

Please do not arrive at the parish before the published starting times. Registration will not start early. Please schedule your plans to arrive during the last hour to avoid waiting in line.
CYC registration dates are scheduled by the parish athletic association. Please DO NOT call the school or rectory for directions.
Please contact your coach or manager. Facilities are in use by the parish until the published starting times. Please remain in your car if you arrive early to a session .Additional (duplicate) CYC photo ID cards may be purchased at the time of registration.
